Life and Accidental Death and Dismemberment coverage is an important part of your comprehensive benefits package. It provides financial protection for you and your family in the event of death or serious accident. ICANN provides you with Basic Life and AD&D coverage equal to 2-1/2 times your annual earnings, up to a maximum of $500,000, at no cost to you.
Long-Term Disability - Mutual of Omaha
This employer paid LTD plan is available to you if you become disabled and remain disabled for 90 consecutive days. The benefit pays up to 60% of your gross earnings, not to exceed the plan’s monthly maximum benefit amount of $10,000, less other income sources. You can be totally or partially disabled during the elimination period.
401(k) Plan
ICANN contributes 5% of your pay to this 401k plan even if you, as an employee, do not contribute. If you do contribute to the plan, ICANN makes an additional match up to 10% of your pay. (For example, an employee contributes 10% of his or her pay and ICANN contributes 5% plus a 10% match, or a total 15%). All employees become vested in all ICANN contributions at the time they are hired.
Flexible Spending Accounts
When you enroll in a Flexible Spending Account, money is taken out of each paycheck and placed in a Ceridian holding account to enable you to pay for certain benefit-related expenses not covered by your benefit plans Health Care Spending Account or, for day care costs, your Dependent Care Spending Account. You can deposit up to $1,500 per year in your Health Care FSA, and up to $5,000 per year in your Dependent Care FSA

Blue Shield Medical Plans
Blue Shield offers a choice of two medical plans: a Health Maintenance Organization (HMO) plan and a Participating Provider Option (PPO) plan. Employees are able to choose the option that best suits their needs. The plan that is best for you depends on your health care needs and how flexible you are when choosing a provider.
Blue Shield Medical HMO Option:
This plan offers a tremendous opportunity to keep your out-of-pocket costs to a minimum since copays apply to services. With the HMO you are required to choose a Primary Care Physician (PCP) to direct your healthcare needs. Your PCP will oversee all your medical care, including referrals to a specialist, if necessary. You must receive all of your non-emergency medical care within the HMO network—and under your personal physician’s direction.
Blue Shield Medical PPO Option:
This plan offers greater flexibility for you to direct your own healthcare needs and a wide range of healthcare providers that can be used whether they are in or out of network, with very rich benefits. As such, be sure to familiarize yourself with the details of the PPO plan concerning deductibles, coinsurance, and out-of-pocket levels for both in- and out-of-network providers. You can achieve substantial savings by accessing in-network providers in the Blue Shield provider network.
Aetna Dental Coverage
DPO Dental Plan
This preferred provider program gives you the option of receiving treatment from any licensed dental provider of your choice. If your dentist is a member of Aetna’s DPO network, you will receive increased benefits and reduce your out-of-pocket costs. If you decide to go to a dentist outside the DPO network, the plan’s benefits will be based on the usual and customary charge for a particular dental service in your area. If your out-of-network dentist charges more than the usual and customary amount allowed by the plan, you must pay the difference.
